We had this issue with Pilot 2017, Code Checking tool was stating a need to replace the Fuel Pipes needing to be replaced. A smoke test revealed an issue with the Fuel Pump/Assembly. Simply replacing the Fuel Pump O ring, and Tightening the Lid on the Fuel Pump resolved the issue permanently.
